In addition, please complete the Open Enrollment Census Request and fax it to us at 614-789-9942.

In completing the form:

• In section 1, use The Pension Design Group as your TPA.
• In section 2, check "Generic enrollment materials".
• In section 3, include your company name and you may complete the financial advisor information as it relates to your plan's investment advisor. When Dave Schafer is your advisor you may enter his contact information as:

• In section 4, Select "Offer All".
• Complete the order in section 5.
• Fax the completed form to Lincoln Financial Group at 260-455-0200.

Monthly Investment Performance can be printed and either distributed with the Employee Enrollment Brochure or Enrollment Booklet, whichever you choose.
